Rosewood Family Health at Gateway is expanding our vaccination capacity with FEMA to help our community get protected from COVID-19. We are able to offer vaccines to adults and kids ages 12 and older. Walk-ins are welcome, or you can sign-up for an appointment with us at yvfwc.com/vaccinateoregon or call us at 503-894-9005.

• Masks and social distancing are required at all medical facilities

Note that submitting this pre-registration form does not schedule a vaccination appointment. This submission adds you to a COVID-19 vaccine waitlist that will alert you when you can begin the scheduling process. Please do not sign up for more than one vaccine appointment per person, with us or any other health care outlets. No-shows can significantly slow our efforts to vaccinate our communities.
