Are you a Hispanic parent or guardian of a child who receives health services from Yakima Valley Farm Workers Clinic? Are you interested in sharing your ideas about the services your child receives and how well these services incorporate your family’s values or the Hispanic culture? You can participate in a study to share your satisfaction about your child’s services by completing an online survey.
You are welcome to participate in this study if you say “Yes” to each of the questions below:
- Parent/guardian identifies as Hispanic.
- Parent/guardian of a child between the ages 2-17
- Their child is enrolled in health services at Yakima Valley Farm Workers Clinic.
- Read and write in English or Spanish.
Upon completion of the survey, you can enter a raffle for an electronic gift card at the price of $50.00. Entry in this raffle is not required to participate in this study. Please click on the link here to participate.
