Yakima Valley Farm Workers Clinic Recognized for Blood Pressure Control
Local clinics earn Target: BP™ Initiative gold-level recognition

Blood pressure control can mean the world to patients and Yakima Valley Farm Workers Clinic was award for the tireless effort healthcare providers have displayed to help patients do just that. Yakima Valley Farm Workers Clinic was the only Oregon Community Health Center to earn gold-level recognition in the American Heart Association’s Target: BP™ program.

One in three American adults lives with high blood pressure, putting them at increased risk for heart attack, heart failure, stroke, kidney failure, and other health problems.

Yakima Valley Farm Workers Clinic joined the American Heart Association (AHA) and the American Medical Association (AMA) Target: BP™ initiative. Target: BP™ aims to help healthcare providers and patients achieve better blood pressure control at the best levels to improve hnealth. Through the initiative, healthcare providers pledged their commitment to improving blood pressure control in their patient population. AHA and AMA will provided information, tools, and resources to help meet these goals.

A total of eight Yakima Valley Farm Workers Clinic Oregon sites were recognized in the American Heart Association’s Target: BP™ program, with six being awarded gold-level recognition. Gold-level recognition goes to clinics that showed 70 percent or greater blood pressure control rate throughout its adult patients.

“Our goal is to create a culture of health where every person has the opportunity to live a healthier life,” Yakima Valley Farm Workers Clinic Senior Director of Quality Lori Kelley said. “To achieve this goal, we consistently seek to help patients improve their overall health and this includes blood pressure control.”

Kelley says the key to being successful is patient engagement and collaboration with a multidisciplinary care team.  Yakima Valley Farm Workers Clinic patient centered model of care focuses on offering services to the patient, all under one roof.

Yakima Valley Farm Workers Clinic sites that received gold-level recognition in Oregon include: Coastal Family Health Center in Astoria, Community Health Center of Clatskanie in Clatskanie, Lancaster Family Health Center at Beverly and Lancaster Family Health Center at Lancaster in Salem, and Rosewood Family Health at Lents, and Rosewood Family Health, in Portland.
