Patients who wish to participate will be issued a MyChart activation code during their clinic visit. This code will enable you to login with your own username and password. If you were not issued an activation code, you can call your clinic or request an access code here.

You can recover your username or reset your password on the MyChart login page. Alternatively, you can call your primary care clinic for assistance.

After your first visit to the clinic, some appointments can be done with a video visit. These can be scheduled in MyChart or by calling your clinic. Learn more about what you need for a video visit here.

You may use MyChart to schedule appointments only with providers who have previously treated you. Appointments with new provider may be made through referral, by calling your primary care clinic or by using our appointment request form.

This will vary by provider and location, though we strive to respond to all inquiries in an efficient manner. Please note that MyChart should not be used for urgent situations or in emergencies. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.

MyChart is a direct link between you and the information in your electronic medical record. It will not reflect your complete medical history, though some patients may see past records in order to ensure continuity of care and access to information about ongoing treatments.

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account cannot be accessed without that password. Further, MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. To protect yourself, do not give out your username and password. MyChart will never send your actual health information via email; however, you will receive communication alerts via email to inform you when new results and other communications are sent to your MyChart.

For your security, your activation code is only valid for a short period of time and is no longer valid after the first time you use it. If you are experiencing problems with your activation code, contact your primary care clinic or request an access code here.
