Hypertension Management and Cardiovascular Risk Reduction: Our services include unique medication treatment plans to maintain your optimal blood pressure control. We also initiate cholesterol-lowering medications based on risk assessments and provide education on lifestyle changes to further your health goals.

 

Hepatitis C Treatment: We provide personalized medication treatment plans for hepatitis C, with monitoring throughout the treatment process. We also offer education on harm/risk reduction strategies to prevent re-infection and coordinate with your care team for lab orders, imaging orders, and referrals as necessary.

 

Anticoagulation Management: Our services include point-of-care INR testing for warfarin dosing, routine lab monitoring for Direct Oral Anticoagulants (DOACs), regular check-ins for assessment of bleeding and clotting risks, and education to promote the safe use of anticoagulants.
