Older adults with chronic medical conditions are often cared for by family members with limited time, resources, and skills. Yakima Valley Farm Workers Clinic wants to find ways to provide for the medical, nutritional, therapeutic, and social needs of these individuals in the community while giving relief to their family caregivers and avoiding the necessity of skilled nursing facility care.

Area residents who are caring for older relatives or those who anticipate their older relatives might soon require more care, are invited to take a few minutes to complete this survey.

“Your input will help us plan better ways to serve you and the people in your care,” said Subbarao Mulpuru, VP of Managed Care at Yakima Valley Farm Workers Clinic.
