YVFWC Community Health Services exists because we realize that "health" does not come simply from what a patient is able to obtain in an encounter with a medical provider.

We provide services in the community to help address the issues our patient population is facing, which ultimately affects their health.

Maternity Support Services

Maternity Support Services enables us as healthcare providers to educate and support you and your family during your pregnancy and after delivery.

Maternity Support Services (MSS) are provided by a member of the agency's interdisciplinary team:

Infant Case Management

Infant Case Management (ICM) services enable us as healthcare providers to educate and support you and your family after the delivery of your baby.

ICM services are provided by a member of the agency's interdisciplinary team. Our goal is to improve the parent(s)/your self-sufficiency in gaining access to needed medical, social, educational and other services.

Eligibility: Your baby must be receiving a medical coupon and be three to twelve months old and meet specific criteria set by DSHS.

Infant Case Management

Maternal & Child Health

Maternal Child Health enables us as healthcare providers to address healthcare needs and concerns regarding your child. A professional can assist you by visiting with your child and you at home.

Eligibility: Your child must meet risk criteria and be under 18 years of age.

Yakima Valley Farm Workers Clinic provides parenting education in Yakima County.

Parenting Classes

Reach Out & Read

The Reach Out and Read program is designed to help parents understand the importance of reading to their children 20 minutes a day. Books are given to the children by their physician starting at 6 months to 5 years of age.

Services are offered in Yakima and Walla Walla counties (Grandview, Toppenish, Walla Walla, Wapato and Yakima.)

Reach Out & Read

Asthma Project

The Asthma Project provides education to YVFWC patients and clientele. The Asthma Project enables YVFWC to offer education to families on the risks, identification and self-management of allergic disease including asthma, allergic rhinitis and eczema.

An Asthma Educator comes to your home to assist the family/client to identify allergy trigger in the home and environment. The Asthma Project can also educate family/clients in the importance of, and procedures for using, medications and medical equipment prescribed to help manage their allergies.

Incredible Years

The Incredible Years program provides parenting classes to teach parents how to foster positive behaviors in their children and to give parents some effective techniques for dealing with common behavior problems. The Incredible Years is a series of interlocking, evidence-based programs for parents, children and teachers supported by over 30 years of research. The goal is to prevent and treat young children's behavior problems and promote their social, emotional, and academic competence.  

Classes are offered in Yakima County only.
