Taking Control of Your Health is a six week, once a week, 2.5-hour workshop for adults with chronic health conditions. If you want to learn tools to manage your condition and improve your quality of life, this workshop is for you.

Chronic Disease Self-Management

You might have a chronic disease, but it doesn’t have to define you. We can show you ways to deal with your condition and feel more confident about taking care of yourself and staying active. Classes are taught in English and Spanish.

Chronic Disease Self-Management can help people with such diseases as, diabetes, heart disease, lung disease, high blood pressure, asthma, and arthritis. We’re also here for the family members and people who take care of people who have chronic diseases; caregivers can learn more about the illness and what the patient needs.
