Yakima Valley Farm Workers Clinic’s Participation in the Making Care Primary (MCP) Model is Ending

The Centers for Medicare & Medicaid Services (CMS) is terminating the Making Care Primary (MCP) Model for all participants effective June 30, 2025. Beneficiary Engagement Incentive(s) and Benefit Enhancements related to the MCP Model will end on this date.  We want to reassure you that, despite this change, the quality of care you receive at our practice will remain consistent and unchanged.

 

Please note, the conclusion of our participation in the MCP Model will not result in a change in your Medicare benefits. Your doctor may continue to recommend that you see particular doctors for your specific health needs, but it is always your choice which doctors you use or hospital you visit. You continue to have the right to use or visit any doctor or hospital that accepts Medicare, at any time.
