Brief Overview
Yakima Valley Farm Workers, in collaboration with Washington State University (WSU) College of Pharmacy and Pharmaceutical Sciences, offers a one-year post-graduate primary care pharmacy residency program for two candidates. Located in Toppenish and Spokane, WA, this program provides advanced training that surpasses traditional academic learning, undergraduate pharmacy clerkships, and general community pharmacy practice. Residents will develop and deliver patient care services, focusing on managing chronic diseases such as diabetes, hypertension, cardiovascular risk, hepatitis C, and anticoagulation. They will also contribute to setting and achieving departmental goals to enhance pharmaceutical services and patient care standards. To meet these objectives, residents will participate in both required and optional experiences within the YVFWC organization. These experiences include primary care services, clinic-based pharmacy management, central fill and specialty pharmacy operations, care for patients with HIV, refugee services, and administrative practices. Additionally, residents will have opportunities to engage in research and gain teaching experience through our partnership with the WSU College of Pharmacy and Pharmaceutical Sciences. Overall, this program emphasizes delivering pharmaceutical care to improve patient outcomes, regardless of the patient's background or circumstances.
Introduction:
- Residency specific duties and responsibilities: - Complete a major longitudinal project - And at least one minor quality improvement project - Create at least one medication use evaluation (MUE) document - Openly communicates with preceptors and assigned residency coordinator about needing support or knowledge gaps before performing patient care services.
- Other duties and responsibilities: - Collaborate with all members of the healthcare team to provide high-quality healthcare services as assigned, which include but are not limited to: - Provision of pharmacy services through relationship based care by in person or virtual appointments -Support of coordinated care at local clinics, including disease state management and population health management. - Provide initial and follow up medication therapy management appointments and provide education for self management. - Sit on local or corporate committees as appropriate: example Care Measurement, Med Management, or P&T - Drug Utilization Review - Patient Counseling - Drug Regimen Review - Medication Therapy Management - Collaborative Drug Therapy Management or other post-diagnostic disease state management, pursuant to a valid agreement - Prescribe a drug or device, according to Drug Therapy Management Protocols - Order, interpret, and monitor laboratory tests - Receipt or transfer of a prescription
- Ensures that they and others they work with operate within the scope of practice and duties of their licensure.
- Supports operational functions of the various preceptors they work with, helping to ensure that inventories and records are maintained in accordance with corporate policies and state and federal laws.
- Supports the security of the pharmacy area including the provision of adequate safeguards against theft or diversion of prescription drugs.
- Ensures all prescriptions, prescription refills, and drug orders are correctly dispensed according to the prescribing provider’s authorization and YVFWC policies and procedures.
- Obtains and maintains active licensure and pharmacological knowledge by attending educational workshops, reviewing professional publications, establishing professional networks, participating in professional societies.
- Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA) and YVFWC requirements.
- Performs other duties assigned and completes tasks and projects on time.
Qualifications
- Degree in Pharmacy, from an accredited program (Req)
- Pharm D Degree, from an accredited program (Pref)
- Experience in community or ambulatory pharmacy practice. (Pref)
- Current State Pharmacy Intern or Pharmacist license
- If not currently licensed as a pharmacist, you must have this credential within the first 4 months of residency.
- Understands the professional expectations associated with the pharmacist’s role.
- Current knowledge of community pharmacy practice.
- Subject matter expert in drug therapy.
- Ability to work as part of a team in a fast-paced environment.
- Current knowledge of State and Federal Pharmacy Laws.
- Strong attention to detail.
- Effective verbal and listening communication skills with a customer-oriented approach.
- Ability to prioritize work while performing multiple tasks requiring attention to detail, with frequent interruptions.
- Intermediate proficiency with a variety of computer systems including Microsoft Office preferred.
- Basic knowledge or ability to learn pharmacy and medical software programs
Working Conditions & Physical Requirements:
- Physical requirements for this position include hearing, seeing, speaking, stooping, kneeling, crouching, reaching, fingering, grasping, and repetitive motions.
- This position requires walking and/or standing for long periods of time and sitting is only required occasionally.
