Better Access. Better Care. Better Outcomes.
“I'm incredibly grateful for the commitment our teams bring to this work every day. Improving quality requires persistence and a willingness to keep looking for better ways to serve patients. These recognitions belong to everyone who has contributed to expanding access, strengthening care, and supporting patients on their journey to better health.”
– Lori Kelley, Chief Quality and Compliance Officer
Community Health Plan of Washington acknowledged Yakima Valley Farm Workers Clinic for meeting quality metrics tied to closing care gaps and improving the accuracy of clinical coding. Yakima Valley Farm Workers Clinic also received a Silver Health Center Quality Leader badge from the Health Resources and Services Administration (HRSA), a distinction that requires multiple year-over-year improvements in quality performance.
Both recognitions highlight the same reality. Investments in access, patient engagement, and coordinated care create meaningful results for patients and communities. Some of these investments include:
- Expanded diagnostic testing allows patients to receive more services in one location, reducing delays and helping care teams make informed decisions sooner.
- Electronic consultations, or e-consults, connect primary care providers with specialists, helping patients get answers faster and reducing the time between a concern and a diagnosis.
- Increased the number of providers offering virtual visits, a convenient option when travel, work schedules, or family responsibilities make an in-person appointment difficult for patients.
- Same-day appointments that help patients address health concerns quickly.
- Provider retention efforts that support continuity of care and strengthen long-term relationships between patients and their care teams.
- A growing network of support services also plays an important role. Patients can work with all members of the care team, such as integrated behavioral health specialists, registered dietitians, and clinical pharmacists as part of their care team.
- Programs such as Tomando provide additional education and support, helping people build the knowledge and confidence needed to manage their health.
Behind every quality metric is a patient who received a cancer screening on time, managed a chronic condition more effectively, connected with a behavioral health specialist, or got answers sooner because care was available when they needed it.
That's what these achievements represent: healthier patients, stronger communities, and continued progress toward making high-quality care accessible to everyone we serve.
