Yakima Valley Farm Workers Clinic is pleased to announce that Greater Columbia Accountable Communities of Health (GCACH) has awarded participating Yakima Valley Farm Workers Clinic sites Exemplar Status for their efforts in meeting quality benchmarks and milestones under the Medicaid 1115 Waiver transformation project.
The four recognized sites are:
- Behavioral Health Services on Mead (Exemplar)
- Toppenish Medical-Dental Clinic (Exemplar)
- Family Medical Center (Exemplar)
- Yakima Medical-Dental Clinic (Exemplar)
Exemplar sites are recognized leaders in improving the quality of care for patients while meeting the GCACH milestones. Yakima Valley Farm Workers Clinic works with The Accountable Communities of Health (ACH) to bring “local and diverse perspectives on system transformation needs and priorities.”
